The quarterly HSTP Region 10 meeting, held at the Flora Banking Company, was called to order at 11:03am, by the Region 10 Chairperson, Mark Auten.  Introductions were done by all attending.

The Region 10 meeting minutes were reviewed and a motion to approve the minutes was made by Betty Cox with a second from Jean Bohnoff.  Motion carried.

Guest Speaker: Dottie Miles, with the Rural Medical Transportation Network, described the Patient Navigator Program, which is currently being tried and tested in many rural areas of Southern Illinois. This program assists patients that may be having difficulty scheduling medical appointments, getting to those appointments, or understanding options and processes get the help they need to avoid further illness. Dottie also discussed the MIH (Mobile Integrated Healthcare) system.  This started four years ago and is helping identify people in certain rural communities that need assistance with various daily living issues and most importantly are either not following through with medical appointments or calling EMS in non- medical situations.  The EMS Navigator does the initial set up visit and discusses with the patient what issues they have (health, transportation, medicine, household, financial, etc) and what is needed to solve those issues.  This will help discourage many elderly from calling for ambulance services, when they perhaps just need small tasks done. Dottie spoke of how often times when patients are discharged from the hospital, they are given a prescription, etc…but no means of follow up with that patient.  Many times elderly do not have family in the same area, that can help them, or if they do get help at home, etc…often times, the worker does not show up.  Dottie stated how their new programs are focusing on these problems and trying to reconnect with different means of help.  Med Trans call center is wanting to branch out to those at home, to help with the many issues that have been discussed.  Dottie mentioned that the Navigator is up and running in Carbondale, Adams County and McLean County.  Facilities from where the patient is leaving can also request the MIH to check blood sugar, weight, etc. and other at-risk. Adam Lach described how Med Trans works in conjunction with both of these programs. MedTrans will set up all appointments with transportation options.  This will help prevent the patient from missed appointments, failing to refill prescriptions, etc. 
CVP Application Discussion: Dennis Shiley began the discussion asking that if anyone will need letters of support, to please contact him so transportation efforts can be discussed. 
Rides & CIPT does provide their drivers with proper OIG training, etc. 
Terri Finn discussed the voting form.  This new form was approved by all three regional committee’s (9, 10 & 11).  Anyone applying for CVP should let Terri know so she can email a form to you.  She will need those forms back at least the week before the May meeting so they can be shared with the voting members for their review.  All voting members must have completed the Open Meetings Act test and sent their certificate in to Terri before the May meeting. 
The next meeting will be on May 19th.  Jean Bohnoff volunteered to host the meeting at the Effingham Senior Center.
Mark Auten asked for a motion to adjourn the meeting, which was made by Dayo Adeneken and 2nd by Debra King.  The meeting was adjourned at 11:56am.

The quarterly HSTP Region 9 meeting, held at Bond Co. Sr. Center in Greenville, was called to order at 11:00am by April Blount, Chairperson.  Introductions and agency updates were done by all attending.
The meeting minutes from December 11, 2014 were reviewed.  A motion was made by Michele Suwe and 2nd by Wendy Settles to approve the minutes.  Motion carried.
Dottie Miles from the Rural Medical Transportation Network was the guest speaker.  She provided information on the Patient Navigator Program and the EMS Mobile Integrated Healthcare pilot project.  Both projects are targeted toward reducing ambulances being called out for non-emergency trips and hospital re-admissions due to a lack of transportation or other barriers. 
Terri Finn addressed the issue of state budget cuts to transit.  $70 million in cuts is proposed for downstate with could increase fares, reduce routes, cause layoffs, etc.  IPTA is advocating for a reduction in cuts and all those involved with providing transportation should also contact legislators.  The committee members had ongoing discussion about various cuts to their programs and services, as well.  Terri Finn will draft an letter that can be used when contacting legislators.  It will include statistics from IPTA.  This letter would be used by Regions 9, 10 & 11.
General Discussion:
·         Voting-CVP applications were sent out, by IDOT, yesterday the May meeting will be a voting meeting.   Any members that have attended 3 of the last 4 meetings and sent their OMA certificate to Terri Finn will be considered voting members.  Endorsement forms will be emailed to all human service agencies and completed forms will then be emailed to voting members the week before the May meeting.  Those forms should be reviewed and questions, comments, etc. should be asked during the agency request presentations.   
·         Terri Finn went over the handouts\resources she had brought for members and encouraged everyone to do some activity for the national Stand Up 4 Transit event that will occur in April.
·         CVP Updates-CY’13 lite duty vehicles are expected for delivery in late winter\early spring of ’16.  Medium duties expected in the spring and super mediums expected in late spring.  Mike Healy has sent the CY’14 CVP award list to HSTP Coordinators so if an agency hasn’t heard if they were approved or not, they can contact Terri Finn.
The next meeting will be hosted by South Central Transit and will be held at their training center in Centralia.
A motion was made by Kent Tarro and 2nd by Michele Suwe to adjourn the meeting.  Motion carried and adjourned at 12:00pm

The quarterly HSTP Region 11 meeting, held at Senior Adult Services in Carbondale, was called to order at 11:06am by Chairperson James Lane.  Introductions and agency updates were done by members in attendance.
The November 12, 2014 meeting minutes were reviewed.  A motion was made by Jesica Gentry and 2nd by Bill Grogan to approve the minutes.  Motion carried.

The Committee had a discussion about the possibility of CVP funding and the application process this year.  Terri Finn informed the group that if it occurs, IDOT is trying to stay with the May deadlines and any information she gets will be passed on to everyone as it is received.  Terri also handed out information on service contracts, discussed planning, communication with the public provider and service coordination.  All members were reminded that if CVP occurs as scheduled, May will be a voting meeting for application requests and all members should make sure they have taken the Open Meetings Act test and forwarded their certificate to Terri Finn.  Also, the new endorsement form will be sent out to all human service agencies so they can get it completed along with the application and forwarded to Terri.  She will in turn forward to voting members a week before the meeting for their review.

Guest Speaker for this meeting was Dottie Miles from the RMTN.  She provided information on the Patient Navigator Program and the new Mobile Integrated Healthcare program.  Handouts were provided and Dottie invited questions and comments.

General Discussion:
·        Voting\Endorsement Forms-Final draft was presented and Terri informed the Committee that Region 10 had approved the final version.  Region 11 then approved this version for use.
·        Voting Members-see above discussion
·        Open Meetings Act Test & Certification-see above discussion
·        Touchette Hospital will host an RTAC driver training on 3\2\15.  Everyone is invited but please call James Lane to reserve a spot.
·        Vehicle update-Medium & lite duty vehicles are tentatively set for May delivery.
·        ‘Stand Up 4 Transit’ is an advocacy event happening nationwide to bring awareness of the need & funding for public transportation.  Terri Finn provided information, flyers that can be used and other items and asked everyone to check out the website and try to have some activities on this day.  She will be glad to post anything going on to the HSTP Facebook page.
·        Information was provided on the National Center for Mobility Management’s Healthcare Access Design Challenge.  Terri Finn and Dennis Presley are forming a team to apply for the $25,000 and anyone is invited to join their team or apply on their own.

The next meeting is set for May 13, 2015.  Shawnee MTD has volunteered to host that meeting, hopefully at their new office in Vienna.
